FINANCIAL ARRANGEMENTS:
We realize that every person’s financial situation is different.
For this reason, we have worked comprehensively to provide a variety of payment options to help you receive the dental
care needed to enjoy a healthy and confident smile with respect to your budget.
Payment
is due at the time of service. For multiple visit treatments, such as crowns,
bridges & dentures, two payments are acceptable with half due at the first appointment and the other half due at the second
appointment. We are also happy to offer a 5% pre-payment courtesy for any single
treatment over $400.00 if paid in full prior to treatment. This applies to treatment
provided by the doctors only and not the hygienists.
Accounts 90 days past due are subject to a minimum late fee of $50.00 and/or of being turned over to a collection
agency.
DENTAL
INSURANCE:
We are happy to file the forms necessary to see that you receive the full benefits of your coverage,
however, we can make no guarantee of any estimated coverage. Because
the insurance policy is an agreement between you and the insurance company, we ask that all patients be directly responsible
for all charges. Please know that we will do our best to see that
you receive the full benefits of your policy.
If for some reason your insurance company has not paid
their portion within 60 days from the date of treatment, you are responsible for payment at that time.

offer monthly payment plans through Dental Fee Plan, including an interest-free option.
